Fall sports season starts in September 2025.
Required Forms for All Student Athletes
- sports physical on file with Justice Page Middle School (form is good for three years)
- transportation waiver on file with Justice Page Middle School (completed each season)
- athletic eligibility statement on file with Justice Page Middle School (completed each season)
Hard copies of all forms are available in the main office. Completed forms can be submitted to the main office.

Make payments
Additionally, payments can be made with a check for $45, submitted to the main office. The check can be made out to Justice Page Middle School along with the sport the student athlete is participating in.
